Job Description

The purpose of this position is to manage the Identity Potential Duplicate report by investigating and appropriately merging patient charts when needed. The position is also responsible for managing the Termed Provider spreadsheet; removing all non-clinical inbasket messages from terminated providers inbaskets, so that the provider can be soft deleted from Excellian. Finally the position assists with performing chart correction cases that have been identified via incoming end user incidents.


  • Manage the Potential Duplicate report in Excellian. Investigate potential duplicate charts by reviewing patient demographic and clinical information.
  • Merge patient records, using the appropriate source and target patients, if the records are deemed to be duplicates. Mark charts as Known Non-Duplicates when the charts are not duplicates.
  • Appropriately identify terminated providers, non-clinical inbasket messages to be deleted. Using Excellian text tools delete all non-clinical inbasket message types. Update the Terminated Provider spreadsheet with the correct number and types of clinical inbasket messages to be completed by the clinical delegate
  • Follow approved Data Integrity policies and procedures to correct documentation errors in patient charts. Create Chart Correction cases in Excellian, use appropriate Identity tools to log and correct errors, and close the Chart Correction case when completed.
  • Contact provider offices to verify provider address, phone, fax and group name. Contact can be made by phone calls, faxes or emails. Update provider information on import spreadsheets.
  • Other duties as assigned by manager.


  • High School Diploma, preferred RHIT or RHIA eligible
  • Registered Health Information Technician (RHIT) or Registered Health Information Administrator (RHIA) preferred.
  • Required 3 years’ experience in a health information/legal setting working with confidential medical information in a fast-paced environment or meets education requirements of RHIT/RHIA eligibility in lieu of experience.
  • Excellent customer service skills (verbal/written) and critical thinking skills.
  • Proficiency in PC use and electronic tools with ability to multi-task.
  • Ability to be detail-conscious and must be able to work under pressure utilizing a variety of databases and electronic systems.
  • Ability to utilize stringent time management skills and accountability of work produced. Experience in a scanning environment is desired.
  • Ability to work with clinical personnel in a professional manner and remain calm and helpful under pressure.
  • Ability to work independently with a moderate degree of supervision.
